Service Fee Schedule for Rewards Cards:

Number of Cards  Fee per Card  1 - 49  $2.00 per Card  50 - 99  $1.25 per Card  100+  $1.00 per Card Visa, MasterCard, American Express and Discover accepted. Cash and personal checks not accepted. Corporate checks are accepted for payment if accompanied by a valid state driver's license.
